Opioid Withdrawal & Medication-Assisted Detox in NJ

What opioid withdrawal feels like

Symptoms mimic a severe flu with heightened anxiety: muscle aches, gastrointestinal distress, restlessness, chills, insomnia, and profound cravings. Onset depends on the substance — 8–12 hours for short-acting opioids (heroin, oxycodone, fentanyl) and 30–48 hours for methadone.

Buprenorphine and methadone in New Jersey

Buprenorphine (Suboxone, Sublocade) can be prescribed by any DEA-registered practitioner in New Jersey following the 2023 removal of the X-waiver. Methadone for opioid use disorder is dispensed only at licensed Opioid Treatment Programs (OTPs), of which there are more than 60 across the state.

Fentanyl considerations

Because fentanyl accumulates in fat tissue, precipitated withdrawal from buprenorphine induction is more common than with heroin. Many NJ programs now use low-dose or micro-induction protocols to minimize this risk.

Naloxone (Narcan) access

Naloxone is available without a prescription at every New Jersey pharmacy and free through many county health departments. Anyone leaving detox — and their household — should carry it.

How long does opioid detox take?
Acute withdrawal lasts 5 to 10 days for short-acting opioids and 2 to 3 weeks for methadone. Post-acute symptoms (sleep disruption, low mood) may persist for months and are best managed with continued MAT.
Do I have to stay on Suboxone forever?
No. Duration is individualized. Most guidelines recommend at least 12 months, and many people benefit from years of maintenance. Tapering should be planned with a prescriber.
Is medication-assisted treatment covered in NJ?
Yes. All NJ Medicaid plans and most commercial insurers cover buprenorphine, methadone, and naltrexone without prior authorization for the medication itself.
