Cigna Coverage for Rehab in New Jersey
Cigna covers medical detox, residential treatment, PHP, IOP, outpatient care, and medication for opioid and alcohol use disorder for its New Jersey members. Behavioral health benefits are administered by Evernorth, Cigna's health services arm, which is a separate phone line and a separate provider directory from your medical benefits. - Medical detox, inpatient and ambulatory, when medically necessary
- Residential rehabilitation, with an initial authorization and concurrent review
- Partial hospitalization and intensive outpatient
- Outpatient therapy and psychiatric care, including telehealth
- Buprenorphine, naltrexone, and methadone at licensed opioid treatment programs
Cigna's behavioral health benefits run through Evernorth Behavioral Health, formerly Cigna Behavioral Health. When you call, ask for behavioral health, and when you search for a facility, search Evernorth's behavioral directory rather than the medical one. Facilities that are "in network with Cigna" for medical care are not automatically in network for substance use treatment.
Open Access Plus vs LocalPlus
Open Access Plus is Cigna's broad national network with out-of-network coverage at a lower percentage. LocalPlus is a narrower, lower-cost network built around specific regional providers, and it covers in-network care only outside of emergencies. For someone in New Jersey looking at a program, the practical question is whether that program is in the LocalPlus network specifically, and the honest answer is that many freestanding detox and residential programs are not. On LocalPlus, an out-of-network residential stay is an uncovered residential stay.
Prior authorization and concurrent review
Emergency detox admissions do not require prior authorization; the facility notifies Evernorth after admission. Residential and PHP admissions generally do. Cigna is known among New Jersey utilization staff for tight concurrent review on residential stays: initial authorizations of about a week are common, with reviews every few days. What extends a stay is documentation of continued medical necessity, which means your clinical team recording what is still unstable, not what is going well. Be honest with them about cravings, sleep, and mood so the record reflects reality.
If your Cigna plan is regulated by New Jersey, the 2017 substance use disorder law requires coverage of the first 180 days per plan year of inpatient and outpatient treatment at in-network facilities without prior authorization, bars concurrent or retrospective medical-necessity review of the first 28 days of an inpatient stay and retrospective review of the first 28 days of IOP or PHP, and bars prior authorization for MAT medications. What you will pay
In-network care applies to your deductible, then coinsurance, up to the out-of-pocket maximum. A residential stay will usually reach the maximum; IOP and outpatient care usually will not. Open Access Plus out-of-network coverage pays a percentage of Cigna's allowed amount, and the balance is yours; ask the facility for a written estimate that assumes Cigna's allowed amount, not their billed charge.
Medication
Generic buprenorphine-naloxone is covered on Cigna's standard formulary, managed through Express Scripts, which is also part of Evernorth. Brand products and monthly injectables may require a step or a prior authorization on self-funded plans. Methadone at a licensed opioid treatment program is covered as a medical benefit. The call
Ask for behavioral health. Confirm coverage at each level of care, your deductible and out-of-pocket maximum and how much is used, whether the specific facility is in the Evernorth behavioral network and in your plan's network tier, whether residential care requires prior authorization, and whether the plan is fully insured or self-funded. Write down the reference number. Cigna also offers a 24-hour behavioral health line that can help locate in-network care in a crisis.
Denials and appeals
Request an expedited internal appeal for any mid-stay denial; decisions on ongoing care come within days. State-regulated plans can go to New Jersey's Independent Health Care Appeals Program after the internal appeal. Self-funded plans follow federal ERISA appeal rules instead, with an external review through an independent organization. Federal parity law applies in both cases: Cigna cannot review addiction treatment more strictly than comparable medical care.
